Meet Elizabeth Stolier, LCSW

Elizabeth is a Licensed Clinical Social Worker with extensive experience working with older adults, individuals with chronic illnesses, and their caregivers. Throughout her career, she has gained invaluable experience in hospice and home health care, supervising personal care attendants and providing grief therapy to bereaved individuals. As a Geriatric Care Manager, Elizabeth’s goal is to provide compassionate support that helps families navigate the complexities of aging and caregiving.

Elizabeth firmly believes that “it takes a village." She sees herself as an integral part of your caregiving team—working alongside family members, medical professionals, and other support systems to provide a holistic approach to care. Her mission is to offer the guidance and steady presence needed to ensure that every aspect of your loved one’s care is thoughtfully addressed, while also supporting the caregivers who play such a vital role.

She understands that caregiving can often feel overwhelming, and her approach is focused on offering clarity and relief during difficult times. By using a client-centered approach, Elizabeth works to identify strengths, empower caregivers, and create personalized care plans that balance both the needs of the client and the well-being of the caregiver.

 Consultation

We begin with a free 30 - minute phone consultation to understand you or your loved one’s unique needs, challenges, and goals. Our experienced Care Managers listen carefully to your concerns and provide expert guidance on available options for aging care.

Home Assessment

A comprehensive in-home assessment allows us to evaluate safety, daily living needs, and overall well-being. We identify risks, recommend modifications, and determine the best level of care to enhance quality of life.

Initiate Plan of Care

Based on the assessment, we develop a customized Care Plan tailored to you or your loved one’s needs. From coordinating medical services to arranging in-home support, we ensure a seamless approach to aging with dignity and independence.
